TALLAHASSEE, Fla. - Floridant -- Chronic care management (CCM) is a preventative service defined by the Centers for Medicare and Medicaid Services (CMS). CCM is generally non-face-to-face and provided to Medicare beneficiaries. Chronic conditions are expected to last up to 12 months or the death of the patient. The main goal of chronic care management is to assist patients while reaching better life quality through their conditions. Remote patient monitoring (RPM); creates care for patient-centered medical homes (PCMH). Specializing care plans for patients allows flexibility and convenience for all.
In a CCM program, a patient may experience a reduction in pain and stress while also experiencing increased physical fitness, improved relaxation, and better sleep at night. Most people get chronic care management because managing their conditions on their own may become too much of a hassle and they need professional help. There are many types of chronic illnesses/conditions. The most common are hypertension, arthritis, heart disease, diabetes, chronic obstructive pulmonary disease (COPD), kidney disease, anemia, hyperlipidemia, and depression. Most of these chronic conditions will force people to change their lifestyles. With a combination of two or more chronic conditions, a person would be eligible for chronic care management, however, having only one will mean that person will not be eligible.

CCM was established in 2015 to combat communication gaps and create better health outcomes for chronic conditions. Since then, WCRx Health has offered live chronic care management services in Tallahassee, Florida, and will combine key elements to aid members in managing their long-term health concerns. If you participate in our CCM program then you will receive around-the-clock access to care management services, help with care transitions, care coordination and enhanced communication, a patient-centered care plan, and an electronic record of the care plan. Our program will provide recorded structured data in the patient's health record, maintenance of a comprehensive care plan for each patient, 24/7 access to care through RPM and PCMH, comprehensive care management, and transitional care management. After enrollment into our live care services, one would be able to receive a continuous glucose monitoring device (CGM) and a wireless blood pressure monitor, a wireless thermometer and more.
