TALLAHASSEE, Fla. - Floridant -- Chronic care management (CCM) is a preventative service defined by the Centers for Medicare and Medicaid Services (CMS). CCM is generally non-face-to-face and provided to Medicare beneficiaries. Chronic conditions are expected to last up to 12 months or the death of the patient. The main goal of chronic care management is to assist patients while reaching better life quality through their conditions. Remote patient monitoring (RPM); creates care for patient-centered medical homes (PCMH). Specializing care plans for patients allows flexibility and convenience for all.

In a CCM program, a patient may experience a reduction in pain and stress while also experiencing increased physical fitness, improved relaxation, and better sleep at night. Most people get chronic care management because managing their conditions on their own may become too much of a hassle and they need professional help.
